Levofloxacin and severe shoulder to elbow pain

Began 1st round antibiotic of Amoxicillin which wasn't working. After 3 days, had Dr call in new Rx of Augmentin which wasn't working. After 5 days, went back to urgent care and that's when the Dr. Prescribed 500 mg peach tablet of Levofloxacin taken once a day for 7 days. Started to bring my fever down on day 3 of it and the Dr told me I would have deep muscle pain while on it. Well, on day five I had deep muscle pain in legs and shoulders and arms. I have been off of the Rx for 3 days and I can't lift or move my upper arms without deep achy pain and popping. Getting some numbness and tingling too. Is this a permanent side effect as a result of taking this powerful antibiotic? I feel pain in my hips as well. I'm scared this drug has caused a debilitating result. Any...

Is it OK to crush the pills of Bactrim and mix them in applesauce before taking? I have taken amoxicilina many times during my life; bought in the US and in Honduras. This last week I got a bottle of 60 capsulas while in Cozumel, to complete my antibiotic treatment (I lost the previous amoxi I was taking). I am very familiar with the smell and taste of amoxicilina and penicillin. I open the capsules when I take it, it tastes bad, and has a unique taste & smell (also in urine). I got this in Mexico last week for a bladder infection. Is this just for the pain of a bladder infection? Or does it help cure one too? ## The active ingredients are 500mgs of Nalidixic Acid and 50mgs of Phenazopyridine, it is commonly used to treat urinary tract infections, because some bacteria that cause them are sensitive to the acid and will be killed by it.
