I have a high (extremely high) tolerance for opiates, I have about 25-30 mcg left of a Fentanyl patch. I know Fentanyl is supposed to be stronger than Morphine however it does not help me nearly as much! I wish so bad that I could go back to MS Contin TR 100 mg three times a day with Morphine Instant 30 my PRN for break through pain. That worked so much better than Dilaudid or Fentanyl to me but I had to change doctors and now I can't get what works best! This leaves me in bed 90 % of the time! This SUCKS!!! My doctors don't seem to think there's much withdrawal if there is med in the pump. I know the change in med will cause withdrawal. Has anyone been in this situation? I've read about handling withdrawal here. I just wonder how this may work and how much the med in the pump will help. ## It really depends on what dosage they are giving you of the Dilaudid, if they make sure it is equivalent to, or a little higher than the amount in your Fentanyl patch, then you shouldn't experience any withdrawal issues. Withdrawal only occurs if a medication like this is completely stopped, without being replaced with anything else. Your doctors should calculate an equivalent dosage and set the pump up to deliver that amount, which is why they probably aren't worried about withdrawal...

I have been weaning myself off Methadone! I was taking 10mgs 3 times a day. I am now down to less than one tablet a day and will continue until I can say goodby Methadone and I never want to see you again. Now to my real problem. Last week my doctor gave me the Fentanyl 25 MCG/HR Patch MYL. I need something to help with the pain I still have, although the Lyrica and Cymbalta have really helped. I did not know when she gave the Fentanyl Patch to me that I would have to taper off and that it carried baggage of bad side affects. I've been getting this for years at Walgreens in Tavares FL and they always have it for me when the day comes for the next Rx but this month they said they cannot order it. They said it's not available from their distributor. Suggested I go to a private pharmacy instead of chain. I called the and they said they would order it and I could pick it up the next day but they didn't get it. Said they can't get it from distributor and substituted a patch made by Apotex. Today I am in withdrawl! Bad. I know that I can't switch generics - they are all slightly different and I am afraid of overdosing from one that is too strong but also hate being underdosed and having withdrawal. My ex-husband has had 5 lower back surgeries and has permanent nerve damage. He also has rods and pins in his back that one screw pushed on his spinal cord and had to be removed-lots of scar tissue. He's having a neurologist for many years. My ex has been on fentanyl 100 patch putting a new patch every 48 hours and oxycotin 5 was 6 times a day now he has been cut to 4 a day. I'm currently taking Trazodone for sleep. I am also involved in a outpatient drug program that uses Zubsolv rather than Suboxone. My last three drug screens have come back positive for fentanyl which I have never used even during my addiction. I asked my doctor the same question yesterday, she said "no", because once it hits the air, the strength starts to dissipate, or weaken. So she put me on the lowest dose, as I hope to get off them and all meds eventually. ## No, you can't cut it in half and the reason for it is because it is time released and cutting the patch could cause you to get too much of the medication at once, which may result in a dangerous overdose. Seven years of back pain with long history of muscle relaxant and more recently oxycodone and lorazepam. Docs finally did MRI and found Spondylolisthesis Grade 3-4 with bulging disc and appears vertebrae have already fused. Still being worked up by Neuro surgeon. Pain mgmt. just switched me from 10mg Vicodin every 4 hrs to 25mcg Fentanyl patch every 3 days with 7.5 Vicodin every 6-8 hrs for breakthrough. Started patch last week. First seemed to work okay but over the last two patches breakthrough pain has increased with increased pain down my legs. I get sweats then chills with pain in all my joints, arms, legs, hands, feet. My husband was in a major car accident a year ago that left him on Fentanyl. He started out on 125 ug and weaned himself over this year down to 25. He decided it was time to get off of the terrible stuff after 2 months on the 25s (with his doctor's enthusiastic approval). So, I'm in a big big dilemma. For my own security and well being, I MUST come off my Fentanyl transdermal patch. I've Been using 50mcg/72 hours for about 6 mths straight. I have 3 of the 50mcg patches left and 4 of the 25mcg to help me taper off. I will no longer be using them anymore, just Norcos or oxymorphone IR. I'm absolutely terrified of withdrawals. My doc who put me on them has No idea what it's like to come off like this. Can someone please tell me if and how bad the withdrawals from going down that much will affect me?
