I don't know where to turn right now so I'm hoping someone can give me some advice. I have been addicted to pain killers for about ten years on and off. Every time I stopped before it was bc I went to detox or treatment. It's been about 33 hours now since I've taken anything n I expected to feel horrible by now. I have a bunch of subs but I'm not sure how long I have to wait to take sub? I'm really in shock that my withdrawal systems are not bad... still, I mean I feel weak n can't get comfortable. Is this normal? When can I safely take my suboxone? Please get back to me ASAP someone if u can. I need to figure this out.

Hi Painwarrior-

I would even wait longer than a couple days. I will tell you why based on my experience... so I voluntarily detoxed off Fentanyl 9 years ago (150 mcgs of Fentanyl patch every 60-70 hours / 400 mcg Fentanyl Actiq lozenges- 3 to 4 daily) and I had been on the Patches for about 7 years. I detoxed under the care of a very specialized Addiction Specialist and this man KNEW his stuff and what he was doing (checked in / visited every day / had in home nurses come by / spoke with my two caretakers daily). Because the Fentanyl Patch absorbs directly into your bloodstream, it is a part of your body. It's a chemical that your body has stored in its cells and all around the body. I took me TWO entire weeks of no patch before there was clinically ZERO trace of the opiate in my body.

DETOX SCHEDULE:
Day One: Patch came off and Actiq intake increased.
Day Six: No more Fentanyl period (done with Actiq) and started Dilaudid every 6 hours.
Day Ten: Dilaudid every 12 hours
Day Twelve: Dilaudid once a day (morning)
Day Fourteen: 1/2 Dilaudid every day
Day Sixteen: no pain meds PERIOD for 48 hours (and that's when things go real bad for me... but I made it through with the grace of God)
Day Eighteen: go to my Pain Management Specialist and begin 1st dose of Suboxone (which I had to take in quarters over a three hour period of time so he could assess any side effects / reactions.)

Listen my friend, trust me when I tell you that bc the Patch distributes that medicine into your blood stream, it takes a long time to completely exit the body. And the longer you've been on them, the longer it takes to purge your body of all the toxins. Think about all your organs just marinating in it for years. It takes WEEKS to completely clean them out and anyone who tells you differently doesn't know their specialty very well. The patches are constantly on our skin as to avoid withdrawals- I used to overlap mine the last 12 hours of each 72 hour period (per doctor orders) bc the last 12-15 hours, I got about 20% pain relief bc MOST of the medicine was already absorbed. So just think about that; we are NEVER without that crap going into our bloodstream. Completely different than ANY other type of med. Equate it to wearing an IV that is constantly pumping meds into your veins. YOU WILL NEED A COUPLE WEEKS for it to ALL be out of your body.

Can I ask you a question? WHY are you taking both of those drugs? No doctor would ever prescribe BOTH the Fentanyl patch AND Suboxone to ANYONE so that means you're getting it on the street or from two different doctors. PLEASE just stick with one doctor for pain management. Mixing drugs without their guidance is dangerous.

I wish you the best of luck and would advise you to NOT mix any narcotics without being under the constant care of a doctor. The withdrawals from the Fentanyl can cause death all on its own- it's why they made me have constant care takers and called / visited daily. And had my mom and husband not been with me 24/7 monitoring my BP and temp and HR, I would've had to be admitted.

Take care friend. Best of luck!
MBK

For 15 years I've been prescribed 150 mcg Fentanyl patches from a very reputable Orthopedic pain manager. My pcp told me he could help. After 24 hours without Fentanyl he gave me half dose of Suboxone. Within 5 min I was thrown into the worse withdrawal hell and to the emerg. room. you have to wait 3-to-4 days and be in full withdrawals before taking Suboxone. Sorry for the news.

I agree. I will lay here in withdrawal and be uncomfortable because I threw myself into withdrawal twice already and I haven't taken anything in 2 days. I still won't touch my prescription of subs as that was by far the worse thing ever. I have been on subs for 3yrs and I relapsed for about 2 months. I had a drug panel come back and it just said 'FAILED FENTANYL'. Today is 2 days and I'm terrified.

I call bulls*** sir or mam. At least in my experience and my brother's, you should wait before taking fentanyl if on suboxone but at most you don't feel it as strong the first day. But I take suboxone before the fentanyl wears off. At least for me there is very little withdrawal. I try to switch at night and sleep through the worst of it. It's so mild I don't even wake up but everyone is different and I only take 2 mg twice a day. Hope that helps. Start low, you can always add but never take it back. Stay safe out there.

I waited 24 after last patch to take a suboxone. Oh my good i needed to wait longer. It instantly through me into withdraws.it was worse then regular withdraws. In my opinion fentanyl is just to strong for the suboxone to even work. But what do i know.

It's already a second pass drug bc of how it was administered. So half the metabolism of it is already done. Also more fluids and any food u can hold down that's light, no red meats...lol, will help speed it up.

Hopeless:

You have many variables in there that affect how your body will react, we don't know when you took the 2 subutex, 2 perk 10's but that yesterday was 1/6th of a sub. Depending on the mg of subs, and when you took the others, you could have compounded the sub in your system by a number we can't compute without knowing. Anywho, regardless, it's not a great idea to keep switching between fent and sub - you'll find your tolerance sky rocketing in no time, trust me. But, it's hard to say if you'll get relief not knowing the amount of sub in ur system or fent you plan on taking. And, you are so very wrong in the statement that you can take subutex without waiting until wd kicks in well, as opposed to suboxone because it has naloxone in it. The naloxone is not, I repeat IS NOT the reason you go into precipitated wd if you dose subs too soon after your doc. Naloxone is merely there to deter intravenous use of the sub, as it's bioavailability is near nilch orally but when taken intravenously, well, we know what happens to an I.V. - o.d. with narcan. Precipitated wd can happen with either subutex or Suboxone because it is the bupe itself that is the cause. If you are not in pretty heavy wd, then that means you still have some of your doc swinging along having a good time on your receptors (even if you're not feeling great, you know it gets much, much worse-so until you are there, you still have doc in your system). So, you decide to take bupe, in either form, BEFORE you are pretty ill from wd, and what happens, is the bupe-who has a binding affinity like Hercules, floods in and literally knocks all your doc that was still hanging around, off of those receptors all at once and wraps them in a protective layer of bupe that won't let anything back on. So now you just got kicked from no-mild wd, to complete worst wd in seconds flat. Which is why it seems so terrible. Make sense? I always try to clear that up, so many people end up sick thinking it's the naloxone, but it is the bupe, I promise.

You can take a subutex and feel opiates whenever the u want but if ur taking regular suboxone I know u gotta wait. But here is my dilemma. I was on sub's since march (this is my 5th time trying to get clean). I ran out of my Subs ten days ago. Since then ive had 2 perk tens and 2 subutex's & 1/6 of a suboxone I took today. For the first six days I took the subutex and I was kosher like a pickle. Went two days wit nothing feeling s***ty, then I took the 2 perks and felt better not great but wasn't going to die. The next day sucked, and the day after that was worse, and today I took the 1/6th of the sub. I'm ok now. I got ten hrs into the sub done already but since it was like less than 2mg of sub with naloxone I should in theory be able to get relief from my fentanyl patch @24 or 36 hrs after my last sub dose? RIGHT?

I have been taking about 4-5 grams of fentanyl daily for about 8 months. How long do you think I have to wait before I take a suboxone? I always take it too soon and it's the worst feeling ever - it's like dying.... I want to do it right & get off this s***! Plz someone hurry I wanna start tomorrow...

Why in gods name would you only take 1mg? Take the whole thing and feel better. You deserve it. You've suffered long and hard I'll bet.

The fentanyl will be excreted in your urine as soon your body begins metabolizing the drug and will continue to be excreted until the patch runs out of 'juice' and your body burns off what remains (3+ days). But, why would you want to apply a 50mcg patch if you are currently on Suboxone? You realize its effects will be greatly diminished given the high binding affinity of Suboxone right? You'd be likely to feel very little if anything from the fentanyl, and that's assuming you are taking a super-low dose of sub and have no other tolerance built.

I am on suboxone. I was wondering if I applied one 50mg fentanyl patch, how long will that drug remain in my urine before being excreted?
