Looking For A Pain Management Dr. In Columbus Ohio That Prescribes Methodone

My son 23, was the recipient of a failed surgery on his shoulder in 2007 , that ate away all of his cartilage, and has oseomyilitus of his arm and shoulder. This was due to an infection on the operating table. 7 operations in 10 days to save his arn and his life ensued. He was on diladuid for 6 years prescribed by pain docs, then his PC Doc. Building up a tolerance to diladuid, and not switching to other opiods eventually turned him to the dark side as he was not prescribed anymore. He wanted a methodone maintenance program, though they are not supposed to take pain patients, though compdrug might in his case, though he needs medicaid to qualify. He had medicaid last year , and they cut him off medicaid, as he did not have a current mri newer than 2010. The current joke is.." they think his shoulder will heal?"!!. Anyway total shoulder replacement is being considered now, as there is no socket to put a partial in. his mom had a consult with his PC doc today, and asked to get him on methadone, get the new mri, and start shopping orthopedic specialist in the field. Many replacements only last 10 years. Imagine a screw into wood after a couple times. it strips. The other issue is The hospital variety of MRSA is still most likely lurking inside his bone , waiting to do it's dirty work and kill him. At this point he will die from buying heroin, using bad heroin, etc. etc. all bad. He has asked for methodone for a year now. I have done extensive reading on the subject. Methodone originally was brought to the United States for morphine dependance after WWII. I say dependance. There is a difference in between dependance and addiction. He at this time obviously has both, though would rather treat for pain, than for addiction for medical reasons, insurance, time,. on and on.. Does anyone know of a pain doc in or around Columbus Ohio that wants to help a young man get his life back, to get healthy, and when he is, to get this very complicated surgery. He told me tonight he is not sure he will make it. What a mess. Any constructive thoughts are greatly appreciated.

That is an unfortunate circumstance. I can't recommend anyone in ohio as I practice in NY. However I will prepare you for what your going to hear. At this time from what you have explained and having not scene his MR or MRI he is an opiate addict. That is going to make it very difficult for him to find a PM that will take him as a patient. I know I myself would not rx him any opioid medications as it is ethically wrong and could very well have legal consequences. However I would recommend looking into Suboxone ( buperenorphine and naloxone) that is probably his only option at the moment. Most surgeons will not operate on a pt in methadone as there are very real life threatening interactions that occur with anesthesia. Also methadone is a very dangerous drug and overdose is a very real possibility. Suboxone is a partial opioid angonist that will give him pain relief and will also assist in his cessation of heroin. Further Suboxone has a ceiling effect whereby after a certain dose taking more will achieve no increased analgesia or euphoria. Some PM groups will take pt in your sons position and tx them with suboxone and other non narcotic pain medications and muscle relaxers. Again I just want to prepare you for a long road as rx your son any opioids is ethically wrong and irresponsible for someone in addiction and the prescriber is liable if your son were to overdose. My apologies I overlooked a statement you made. You said he isn't sure he will make it. I implore you to seek psychiatrist help immediately. That statement is indicative of someone contemplating suicide. Addiction is as much mental as it is physical. I cannot impress upon you the importance of sharing that info with his PMD ASAP.

Thanks for your reply, from NewYork. Since I last wrote, my son had a new MRI, and subsequent surgery, that was partially successful. The MRI showed significant flattening of the humeral head , and thinning of the glenoid. A humeral head prosthesis was implanted, and that should help a lot. the surgery was performed late November of 2013. It will be a long road to gain any use of it, and at this point he can not lift the arm by itself, though that will come. In preparation for surgery his PC physician did put him on methadone for pain, and after surgery it was discontinued, via detox, though as you know it is a much longer half life. They should have kept him longer. He had post surgical meds, and we never found a pain group in Columbus, so he is at a pain clinic that uses subutex(helps with pain). He has had an acupuncture treatment thus far, and there are other pain treatment alternatives being explored, along with continuing physical therapy. This was about the only option left for him. He has been at the clinic about a week now. Any suggestions are helpful.

I am very sorry to hear about your son and i would like to just say that I know first hand what you are going through. I myself am in severe chronic pain that will only continue to get worse and i to am a addict. I get very upset when I hear that people will not prescribe to people such as us because of our history. I do not understand that in any way. I feel that we should have the same rights as anyone else has when it comes to getting the proper care for our medical problems. If we was to overdose we could do it several ways. I only took pain meds before my injuries but with the lack of medical care and the extreme pain the only choice I was left with was to go to the streets. Buying pain meds was to expensive so that left Herion. I don't feel "high" off Herion and that is the honest truth. I do enough to be able to walk. If I do not take anything I am unable to do any of the normal things that are easy for others to do. Also suboxen is used for moderate pain and in my experience does not help with pain. Subutex will help ease the pain for a little while but he will still have the pain and yes it does make you suicidal. I think about suicide very often. As someone replied to you that she would not prescribe to your son because he is a addict is the reason that many end up on the streets buying drugs and ending up overdosing or taking something that they have no idea what they are taking. If more doctors would truly understand addiction and understand that people in our situation would do whatever it took such as weekly appointments pill counts drug test what ever the doctor wanted just to go a day with out the extreme pain we would do it. As a doctor who takes a oath to help others it just seems inhumane to me that because of some mistake we made in the past or even if current that we are not able to get the medical help we need not want need. Your son is going to think about suicide and its going to be a very common thing for him because we feel like there is no hope because of the ignorance of those who will not give us a chance but just encourage him it will get easier and one day there will be a Doctor who is smart enough and will take the time on him. I have to remind myself of that daily as I still have not found that doctor. Although all the doctors I have talked to say I need to be on methodone as it is the only thing really that will help with the pain and give me some kind of comfort. if I find a clinic I will surely let you know as I am going through the same thing you all are right now. Again I am so sorry to hear about your son and I know that it not only effects him but you as well. There has to be someone out here that will understand just keep reaching out as I am and we will find the answer one day.

He is theeee most caring man/doc. that Ive EVER had or known. His staff sucks sometimes but he gives me the proper treatment Always. I'm sure he will benefit your Son immensely. Get him to a good stem cell therapy clinic, like Regenexx
Forget pain mgmt they're utterly useless, not to mention outrageously expensive. Some addiction tx centers might be willing to help him by rxing, as one other person responded, with Suboxone. But avoid doctors who tx both pain and addiction because they're more likely to pull the rug out from under him.

It's really the fault of policy-makers Who are only going to make the problem worse by forcing legit pts you find pain relief elsewhere.
