Nurse Laziness?

I just had my spinal revision surgery on 11/4/2016, and due to the severity of the issues and problems, the surgery took far longer than expected. This surgeon was expecting, and correct, that I would require a stronger pain medication regime following surgery. Through several trial and errors, and the need to be off of IV therapy to be admitted to rehab, we decided that I would be on ms contin 30mg XR and dilaudid 8 mgs x4hrs prn. Pain management working well with only minor breakthrough pain easily handled through controlled breathing. I have found that a dose of ms contin at 8pm followed by a dose of dialaudid at 10pm allows me somewhat decent sleep until 4:30-5:00am! Something that I haven't been able to do for at least 6-8 months! Until tonight of course!! Nurse gave my MS contin at 8pm as usual, but by 9:45 I was still in excruciating pain and when I asked her for my dilaudid at 10 she said, "oh I already gave you your last dose for tonight". As you can imagine I was not too happy for a couple of reasons.

1. Shouldn't I have been informed of the change?
2. This now decreased my pain control by 2 hrs
3. Aren't these 2 meds notoriously not given together bc of health risks?

Oh and the most important info regrading this situation is that she actually wrote a nsg note stating that I was offered the option to be taken to the the local ER for pain or choose to take these meds together. NEVER happened!
