How To Ask For Ativan Refill?

I'll try to make this as short as possible.
How can I ask or hint at that I need a refill without sounding like an addict?

I'm suffering from Agoraphobia, my Primary Doctor put me on Ativan, side effects suck but it works good enough. He gave me the prescription for the 3 months (I live in Florida) & that was in June of 2016. Yes, it's lasted me over a year. I take 1.5mg to be able to leave my home. I take them maybe a max of 4 times a month. That's leaving my home once a week. (1.5mg once a week basically)

I've made an appointment w/a therapist back in march but can't get in until September. I'm waiting! I don't have enough medication to last until my Therapist visit, unless I stop leaving my home for the next few months. That makes things worse if I don't push myself to leave, so that's not good for me.

I'm seeing a new primary doctor today. My old primary is no longer in my insurance network and he's expensive. So that's why I'm freaking out wondering how can I ask the new doctor to help me. I have no other "controlled substances" meds I take. (Vit D & Birth control pill is all I'm on) should I ask for/hint at Anti-depressants instead or??

My previous Primary said it was fine to take them as needed & not every few hours. Yes, I've tried not taking them & leaving my home, not good! For those who aren't familiar w/Agoraphobia-my attacks happen unprovoked. I won't know when or where they'll happen, I'll go into a full panic attack (chest pains, body shaking, "heart attack" mode). There aren't places to avoid, everywhere & everything could trigger me. Thank you for any help! :(
