25 Mg Fentanyl Patch

I am currently wearing the Fentanyl 25 patch. I have had some (4) bad experiences this month alone. Exerting myself - gardening, carrying groceries, lifting anything; I become short of breath, extremely rapid heart rate and my legs and upper body become very weak. Has anyone else ever experienced something like this. I honestly thought I was having a heart attack. To be fair, I had awful leg weakness before I started taking or using anything. I don't know what to do. I took an OxyContin (30) mg and had an even worse experience than mentioned above. (Shortness of breath & rapid heart beat went on longer). My pain doc. changed me from 30 mg methadone 3 times a day to a 25mcg fentanyl patch every 48 hours since I started I have had nothing but hot and cold flashes / withdrawal for 3 days and almost zero pain control does anyone know what the conversion rate of 90 milligrams methadone per day vs 25mcg fentynal over 48 hours is? I sit with a 73 year old lady. I take care of all her meds and refills. I thought she had a refill on file...long story short,she didn't. She is on 25 mg patches. Her patch was supposed to be changed this morning at 9 but they wont be ready til mnday morning. Is a 25mil fentanyl patch as strong as 40 mil of methadone? ## From what I've read online, fentanyl is reportedly much stronger than methadone. But because the forms of administration are two worlds apart (one being a transdermal patch and the other being a pill), I honestly don't think there's such thing as an "equivalent dose" between the two; especially since each medication effects people differently. I'm in Massachusetts and just lost my Blue Cross that was covering the Duragesic Jansen 100 and 25 mg patches. Now I'm on Massachusetts Health and they won't cover the brand name, which compared to the generic is a big difference in the amount of Fentanyl. For example the 100 has 16.8mg of Fentanyl per patch vs the generic only has 10.55 by mylan. The 25 only has 2.4 I believe and the brand name is 4.2. That's a big difference to me when these name brand are barely making my pain tolerable. I've been on the generic before and they are definitely not even close to the brand name. I have a patch that is called Transdermal system and it says 25 mcg/hr, it also says each Transdermal system contains: 2.55 mg Fentanyl. I am a first time user on this medication, what effects will I experience? ## Well, this can have some adverse effects. I am currently using 125 MCG fentanyl patches - 1 100mcg and 1 25mcg for chronic pain due to fibromyalgia and Addisons Disease. My orthopedic surgeon just informed me today that I have a large bone spur and a torn rotator cuff on my right shoulder. I have an appointment on Tuesday to see my pain mgmt specialist and I can discuss this with him but often he has mislead me with instructions on how to take my meds and vitamin supplements, one time landing me in the hospital in full blown Addisonian Crisis. No, you should not take a medication containing Hydrocodone or any other opioids with your Fentanyl patch, as they could result in an overdose. Im on morphine 20ml daily and fentonyl patches 100mg every 3 days I ran out of my pain relief last week n used subutex today. I have been on Fentanyl for a long time. 3 weeks ago I kept a patch on for over a week without changing it and then took it off a day before going to the doctor. I had sweating, nausea, pain but I tolerated it. My doctor was upset that I did it but I was so desperate to come off of it, I would try anything. She put me on 12 mg patches for 2 weeks and then I was to go onto something else. I was having a horrible time with the low dose patch and put on a 25 mg 2 days before seeing my doctor. Seven years of back pain with long history of muscle relaxant and more recently oxycodone and lorazepam. Docs finally did MRI and found Spondylolisthesis Grade 3-4 with bulging disc and appears vertebrae have already fused. Still being worked up by Neuro surgeon. Pain mgmt. just switched me from 10mg Vicodin every 4 hrs to 25mcg Fentanyl patch every 3 days with 7.5 Vicodin every 6-8 hrs for breakthrough. Started patch last week. I was on liquid Methadone for chronic pain for years and while on waiting list to switch to a new Pain center, I was weaning off methadone rapidly at 2mgs a day from 120 to 38mg. I was also on 20 to 40 mgs a day oxycodone for breakthrough pain. Now, last month, my new doc put me on 25mcg/h patch. Yesterday, he informed me that I am now ONLY getting pain management from him (understandable) and cut me off and did not prescribe any more oxycodone and I was to stop at 38mgs methadone.
