Fentanyl 75 Not Enough. Will Pain Doc Increase Dose?

These past weeks have been very difficult pain-wise. I fell on ice and injured major joint. I am prescribed Fentanyl 75 (matrix) patches for my DDD/DJD, one every 48 hours. I've cutting one up into quarters to get additional pain relief. I will often use 1/4 extra patch, in addition to the one full patch. Now I cannot tolerate anything less! My pain is bad, but now I'm going to be a little short before next refill. If I tell my Pain Management Doc about this on my appointment, will he kick me out, or will he help me? What I really need is a higher dose for my pain.

Hello, DNvest! How are you? I'm very sorry that you're in pain.

It really would have been best to inform your doctor ahead of time that your current dosage was no longer handling your pain. There is no way that anyone can speak for your doctor, we can only guess at what they may or may not do.

Do you have a pain treatment contract with them? If so, you might want to look it over and see if you're in breach.

The FDA lists this medication as being a narcotic analgesic, so it has the potential to be habit forming and may cause side effects, such as nausea, dizziness, drowsiness, dry mouth and constipation.

Does anyone have any ideas or advice to offer?

thak you for your reply. Yes, I have a contract, but i don't recall where I placed it. I will find and review asap. I might as well just come clean with him. My appointment is in a few days. I've only been seeing him for 3 months (transferred from my rheumatologist of 10 years) and there were a few mixups with my scripts that caused delays that he accidentally caused and is aware of. These delays contributed to my extra pain and predicament. Regardless, I am only 2 days short of 30-day supply of the current prescritpion of patches. It's not like I'm gobbling them up. Hopefully he will understand and adjust my doses to ease the problem.
