Hi, Carolee! How are you?

There really is no name for this, it's something that the doctor specially orders according to the person's symptoms and the pharmacy basically creates it for them. The numbers are the dosages.

The cyc stands for Cyclobenzaprine, which is a muscle relaxant. You can learn more Cyclobenzaprine details here.

The bac stands for Baclofen, which is another muscle relaxant. You can learn more Baclofen details here.

The gab stands for Gabapentin, which is an anticonvulsant that also helps with nerve pain. You can learn more Gabapentin details here.

The keto stands for Ketoprofen, which is a nonsteroidal anti-inflammatory. You can learn more Ketoprofen details here.

But on the last one, the tra, I have to admit that I am not sure, because there are several different possibilities, but if you call the pharmacy, they should be able to tell you.
