It means that you should take the recommended dose once every 12 hours. So if you took one dose at 9 am, you should take the next dose at 9 pm. Does that make sense?

It depends on the drug. Is it time critical such as a diabetic med or an antibiotic. Diabetic med dosing is for the most part time critical. Take or dose at the prescribed interval. An antibiotic not so much. You will not die if an absolute dosing regiment is not adhered to for an antibiotic.
