For adults under 65, Doses of 30 mg every 6 hours NOT TO EXCEED 120 mg in a 24 hour period.

For patients over 65, renally impaired, or less than 110 lbs (50 kg), the doses should not exceed 15 mg every 6 hours, and less than 60 mg in a 24 hour period.
