Rare Gram Positive Cocci

One of my cultures from my gums returned with rare gram positive cocci and I want to know what it symbolizes as well as anymore information about this result?

Gram positive cocci are a group of bacteria which can be identified by their shape and Gram Stain color.

According to wikipedia, "a coccus (plural cocci) is any bacterium or archaeon that has a spherical, ovoid, or generally round shape. It is one of the three distinct bacterial shapes, the other two being bacillus (rod-shaped) and spiral-shaped cells."

A cocci might be referring to:

- Enterococci (UTI, stomach infections, bladder infections, etc... Bacteria normally found in the feces.)

- Staphylococcus (e.g. staph infections... boils, impetigo, food poisoning, cellulitis, and toxic shock syndrome)

- Streptococcal (e.g. strep throat... a bacterium often found in the throat and on the skin)

With those examples listed above, I wonder if you have recently had a throat or respiratory infection, or maybe food poisoning? There may be other cocci that I haven't listed, but I just wanted to share those examples for reference.

My ENT Dr. and I have been together 20 years and has performed successful surgeries and diagnosed infections that were difficult for the other Dr.'s. He will be performing another biopsy on my large painful lymph nodes down the left side of my neck, a bump on my left mastoid, plus lesions in my mouth. My extensive researching lead me to articles from National Institute NIAID where I came up with Epstein-Barr virus articles. I was diagnosed by this particular doctor 25 years ago with chronic fatigue.

I believe next week after biopsies of the lesions on my inner lip, and a test from my swollen glands we will see some results.

Hi Laura, Thank you for your feedback. From your recent post, it sounds like things are going in the right direction with the upcoming evaluations. This ENT Dr. of 20 years seems very promising in his attention to detail so I am under the impression that you're in good care.

In my research, sometimes swollen lymph nodes are related to viruses, bacterial infections, allergies, or simply a lymphatic system blockage due to the body needing detoxification and more exercise.

However, I can see why "Epstein-Barr virus" comes up in your investigation, since its symptoms correlate with swollen glands in the neck and fatigue.

Hopefully the biopsies come back with an answer that you can easily act on and address. My thoughts & prayers are with you and I look forward to hearing how things go.

Regarding the rare gram positive cocci, could that be from an active EBV virus? My blood work is EB early antigen (EA) IGG 103. EBV Nuclear AG (EBNA) AB (IGG) 388. EPB virus EBV IGG VCA >750. EBV VCA AB (lgM) <10. What antibiotic is suggested? We did no further testing regarding the Rare Gram, . what does <10WBCS seen mean, <10 squamous epithelial cells seen.

Hi, I am now returning to the hospital for the third time in one year. My stomach has thickening.

I am having abdominal surgery after a CT Abdomen and pelvis w/contrast indicated somewhat thickened wall of gastric antrum, which could be peristalsis (cannot entirely rule out the possibility of wall thickening due to pathologic process. Large bowels are distended with excess amount of stool, transition at the anastomosis site of rectosigmoid junction and soft tissue of the rectum which raises concern for possible partial obstruction. Thickening of the gastric wall along the antrum could be possibility of mucosal pathologic lesion. Will my Positive Rare Gram Staph. cocci include any preventive course of action prior to surgery, during, and after?

In short, my answer is yes, it theoretically could be from an active EBV virus. There are actually many connections between EBV and chronic illnesses in general.

On a related note, "EBV" is a virus and "gram positive cocci" are bacterial - two different types of entities, potentially putting into question whether a viral infection can cause a bacterial infection? And is it possible to have a virus and a bacterial infection at the same time? My research says yes, but being co-infected with two different types of infection, bacterial and viral, can lead to serious problems. Furthermore, in certain circumstances a viral infection can 'turn into' a bacterial infection (e.g. if you have congestion that lingers too long, it can get infected).

According to the Centers for Disease Control and Prevention at cdc.gov, the "Epstein-Barr virus, or EBV, is one of the most common human viruses in the world. It spreads primarily through saliva. EBV can cause infectious mononucleosis, also called mono (characterized by pharyngitis, cervical lymph node enlargement, fatigue and fever), and other illnesses. Most people will get infected with EBV in their lifetime and will not have any symptoms."

I'm honestly not too familiar with the readings & measurements of your blood work in post #5, but wanted to address your most recent question in post #6 asking the following, "Will my Positive Rare Gram Staph. cocci include any preventive course of action prior to surgery, during, and after?" - In my personal and humble opinion, the best course of preventative action prior to surgery, during, and afterwards should entail a focus of strengthening & supporting one's immune system day in and day out, by making healthy decisions when it comes to the foods we put in our bodies, thinking healthy thoughts when it comes to our perception of reality and our state of being in a moment to moment basis so to speak (i.e. reaching for better feeling thoughts & words we speak to feed our mind/body with), and continuing one's due diligence with our physical fitness in whatever capacity that seems suitable to our own unique levels of health & conditioning.

I understand that this is a very complex matter as a whole and there are many facets by which to measure or consider when trying to grasp a rational perception of everything at once. So as I wind down for the night, I just want to wish you a joyous and very prosperous 2019 as we begin to welcome in the New Year. ~Here's to our health!
