It does indeed look like a fish, but it is supposed to be an R, it's the logo used by the manufactured Actavis on some of their products.

This one contains 1mg of Lorazepam, the active ingredient in Ativan, it is a Benzodiazepine that's most commonly used to treat anxiety and nervous disorders.
