400-600 Mcg Fentanyl Need Help

I was in a serious motor cycle accident 10 months ago and severly broke my leg very bad compound, steel rod, plates, the whole nine yards and broke two vertebrea in my back one crushed on 90% broke. Waiting for spinal fuse surgery. started off with oxycodone then oxycodone and oxycontin then finally up to2 x 80 mb oxycontin and all the 10 mg oxycodone i could take, it just didnt work anymore. So my doctor put me on fentanyl 2 -3 patches every 3 days. i am going to see a pain specialist in a few days. The Fentanyl work OK but the he still give me that stupid oxycodone ir for breakthrough, i could take 10 of those at a time and will do nothing. What should I ask my pain specialist for. I just started a new job in a new city and have to keep working, I have3 kids putting through university so I have to put the surgery off for a year.Without the pain meds I cant even move. now I am so tolerant they are not working well anymore. Will I get immune to the fentanyl becuase it is not working as well and what should i ask him for breakthrough pain, when its bad i cant move, I look like a complete idiot at work. my job is a very high professional position and I really need it. Can anyone help me.

I would talk this over in depth
with your doctor or other health care professional. You might want to try and get the surgery right away. This would decrease your depending on the drugs. Just a thought. But seriously, discuss this with your doctor.

Your body will adjust to any medication that you use regularly for a long period of time, including narcotics such as Fentanyl.

I am not certain what dosage you are on, there is a 100mcg patch available, so you should discuss this with your doctor as you may need a dosage adjustment.

Your best bet is to really consult with the pain specialist and follow their recommendations.

I red your post, obviously you are in serious pain. Here is the thing. You are going to adjustto any oiod medication. Actually, the does of Oxycontin was kind of low. Believe it or not, my pain Dr. had me on 6, 80MG per day, plus 4 Percocet. This was back before Purdue Pharmacutica got busted for putting an ingredient in the Oxycodone that caused people to want more and more. (They actually lied to a congressional panel regarding this issue. The CEO, CFO and CMO were all fined) On this much Oxycontin, I do not remembe 2003 or 2004. As for the patch, it works, but the big problem is keeping them stuck on.If it falls off, and you re-apply it, IT WILL NOT WORK. Think about it. It is designed to penetrate thru your three layers of skin. The dead skin cells produce a fourth layer of skin. The bad thing is, the DOctors (at least in my state (Texas) are so watched by the DEA that 99.9999999999% of them will not write a script for replacements. It is a shame and a true problem and failure of the device to adhere to the skin, but they don't help much.

Here is one trick though. (A pain Mgt Dr. gave me this little jewel of advice. Ask your family Dr. for a scrip for a spray nasal decongestant. (They are designed to open the nasal membranes and have the same effect on your skin!!! I use Flonase. That is all I can think of

Thanks but you didnt answer my question, I already know all that about fetanyl thats why i wanted to ask my doctor. You didtn answer my question, but thanks anway...

You might look into diff. forms of fentanyl such as a buccal form onsolis, and a pop cicle form called Atiq to supplement the patches. Let me know what you think.

Actiq is a great help for chronic pain and did not make me sleepy. Problem is insurance company may not cover if you do not have cancer as it was created specifically for cancer patients. BCBS used as an excuse to quit covering. No other combination of meds has worked as well over last 2 yrs. of exploring options. But then ins. companies are not interested in quality of life, just dollars.
