it is an oblong oval shape, about as close to the standard pill form you can get. the top and bottom are slighty rounded and the sides are straight. the only imprint it has is a 315 on it, back side is blank. ## Physician's Total Care lists this tablet as being a Vytorin tablet, it contains the active ingredients ezetimibe, and simvastatin.
