Sorry you're so miserable. Vertigo can be quite awful to deal with. If I had to make a guess, I'd suggest that the most likely culprit is the Fentanyl, since it's a very potent opiate narcotic, it's over 80 time stronger than Morphine. "Percocet's most frequently observed non-serious adverse reactions include lightheadedness, dizziness, drowsiness or sedation, nausea, and vomiting."
