side effects of dilaudid and fentanyl 150 mg

My brother has Stage IV lung/bone cancer and the new pain cocktail is 8 mg. dilaudid and 150 fetanyl patch plus IV dilaudid every 2 hours. He will probably be released from the hospital today or tomorrow so they have been easing off the injectable dosage and administering it oralls. The increased dosage of the oral dilaudid is causing him to vomit, which will lead to dehydration etc. What is the next best solution to solving this insidious problem of keeping his pain under control without these side effects? I have had an back injury for many years and need help finding an doctorr that can help me keep my buprenorfin-dosage on a decent level. I have had a very good relationship with my doctor for over 12 years but now he is going to retire becasue of age. He is getting 83 years this year. Whole thing is very unfortunately since I have had him for 12 years but this is going to be roughand I feel I need help with solving my solutiion.
