What is the conversion for sublingual tabs to a butrans patch? I was on 20mg tabs and have now been given 20mcg patches. I'm on hour 24 and just not feeling great. I can't find much info on this switch. I've been taking 45-60mg of oxycodone daily for quite awhile now. My doctor would like me to cut down to 45mg (3 x 15mg) more often and add 10mcg/hour buprenorphine patches and only take 60mg (4 x 15mg) as needed. I am currently using Norspan patches for the treatment of chronic pain, I am moving to Beijing China and dont know if I can have this medication prescribed in China. I've been taking 30mg of Methadone for a few months. I've had 3 severe back surgeries and started taking the Methadone until I could get into pain management. My doctor put me on butran 10 mg patch. I got my rx filled and received 4 patches. The first on fell off the first night. The second one worked great for the 7 days. The third patch got wet so I had to change it. Now I have the last patch on. It has been 24 hours and it's still on but having no pain relief any advice ## It seems odd that one would work, but not the other. It would make more sense if none of them helped you, at all. I've been on 150mcg Fentanyl patches Q48 for over 8 yrs. 7 months ago we reduced it to 125 mcg Q48 i only had the chills for a few days. 6 weeks ago we lowered again to 100 mcg. and I am having trouble sleeping, i cant turn my mind off and i cant stop talking?
