I think I was unclear in my previous post. We have a condo in NYC so I don't die prematurely of stress- and traffic-related PTSD! My practice is in NYC and my hospital position just a little bit out on Long Island (it's 120 miles, so "a little bit" means 5 miles from the NYC border). This accident slowed me down a lot and that I had to use my own resources to get the pain meds I needed infuriated me. Fortunately, I"m old enough to have friends who were Board Certified anesthesiologists and EXEMPT or GRANDFATHERED fin re: these new regs. I got spoiled, since shrinks always have been given a lot of leeway with Controlled Substances. I treat our addicted patients with respect and dignity and my Sicilian temper goes into high gear when I hear a pain patient isn't getting properly medicated. I don't believe a "legit" pain patient is superior to an accident. I had migraines for years and scarfed down a lot of pain meds and benzos. Legit? Marginally, but I believe I can feel the pain of both addict and pain patients. I don't have patience for its trying to game the system and feel high while in a Drug Treatment Program. I see a lot of this on these forums. On methadone, but asking Klonopin can I take the week before my U/A? Some pts need maintenance meds, addicts or not. I have the licenses to write for some restricted drugs and, when necessary, I sign off on my Residents' orders.

Unless you presented on your first visit while having a serious Hypomanic Episode, I don't think I'd do a Ddx in five minutes. In fact, I know I wouldn't. The few residents I heard of who were doing this met "The BAD Dr Joey" (a best friend and colleague, a pediatrician in charge of the NICU said to a resident, "You don't wanna meet The BAD Dr Joey — do as he says. And she's senior to and has better creds than mine; she's also built like Lynda Carter as Wonder Woman: 6' without heels!) That's irresponsible medicine by any measure. I know how fear can stop us from doing what we really want to do. It's happened to me. I don't mention my past addiction except to drug/alcohol patients, but I won't lie if directly questioned (I"ll probably ignore the question. After a couple times, they get the message. For all my education, I can use the English language in its most graphic terms. "F***ed if it's any of your badness: works, too.)

@Dr Joey OMG you and I would make a hell of a team LOL. Your posts are beginning to be one of my fav things. You and I broke the molds, I was always one of the smartest kids in the class in fact I was going to law school but after hearing the horrors of the LSATS I chickened out and went for business, I really like the fact that you treat the LGBT community. I have some really good friends in my town who have finally gotten married and we have a pretty big gay community. Our downtown is full of artsy stuff and all kinds of galleries all within walking distance. But very little treatment for all of the inner city problems. We are an hour from NYC and an hour from Philadelphia so the gangs and dealers make the hour run drop off the goods and come back and pick up the loot. And we are left to pick up the pieces of the broken mess. Monthly waits for shrinks Mental health is a joke and the homeless rate has skyrocketed. So where do they turn, Drugs. Your posts give an air of hope and sound advice. How can you not respect a Doctor who has openly admitted to being an addict and who hires addicts. Not here in the Lehigh Valley of Pa. All of the treatment in Allentown is either medical assistance or cash. And with the DEA on the mission to control your practice no one will treat addiction. Hell Pa does not even have medical marijuana but you can cross a bridge where I live and be in New Jersey where they are starting some clinics. They have plenty of methadone clinics so a lot of people go there but it is a chain of for profit clinics and very expensive. I ask why and the rationale is if you can buy dope you can pay for methadone. I was also diagnosed by a shrink as bi polar after 5 min into the session. I ask him why he thought that and his reply was he could bring up a subject and I could discuss it. Now where did he go to school?? Buspar and I seem to be doing well together after my car accident I developed PTSD and fight it on occasion.Yoga helps and meditation,Early on I found exercise released those endorphin's so that is a part of my daily routine. Eating healthy is also a good rule, try and stay away from caffeine, it just makes stress worse for me. So those are my thoughts for a rainy Sunday morning. peace and to all of those who are still searching for the miracle cure. Look within, we all need help for your sake and those who love you get it.

I'm not retired, but have been out for close to a year in the aftermath of a really bizarre accident. (Took place in a parking garage and no cars were moving or even their doors' opened!) I'm from the Albany-Troy area in NYS area, comfortably away from NYC, but I'm spoiled. We have a condo there so I don't die prematurely of Aggravated Drivers' Anxiety & Traumatic Stress Disorder. Thanks for the kudos. I was always too smart for my own good when I was in high school and college.

@Sick and Tired: Thanks for your condolences, bud. My Mom and I were very close. I've had more than my share of incredibly good-looking boyfriends some serious (a couple of CK models, actors, etc.), but she inspired me to use the brains she and God gave me to do some good in the world, not just make money. I"m sorry you lost your parents at such an early age. Talk about loaded dice, my friend! I lucked out again when my Mom wouldn't go along with the AA/NA hysteria when I had a pill problem. I got help, from a shrink who became my mentor. He and out family Internist (who had more specialty certs than even I do!), started asking me about medical school. Doc F said that we need more shrinks who know how to use the English language n all its forms rather than just write for pills. (I was on my way to a PhD in English at this point.) I finished that degree because I really love writing and literature, but I also took a "year off" program to ensure I had all my creds for apps to med school. I"m a good test-taker; I've always been, so I got into my first choice med school and from there, away we went! (Remember Jackie Gleason?) Incidentally, my shrink was the Face of clinical Psychiatry on Long Island. He thought there was some underlying Depressive Illness, but "I can't treat what MIGHT manifest," so he waits until I showed ONE miserable symptom and then shared his theory. He was right. I take Prozac, Effexor, Klonopin [always had significant Generalized Anxiety Disorder] and Ritalin [because my Major Depressive Episode had to be tx-resistant —of course! I'm careful with my meds. I won't use the old trick of putting my sister's or boyfriend's name on my scripts. My meds come directly from my shrink's successor (and my good friend). It's insurance against thinking what happened once, especially with amphetamines, cannot happen again. Yes, some pts with Hxs of addiction NEED potentially addicting drugs, but it's up to me to monitor them and I do a pretty good job. I see patients with other psych issues as well, of course and I"m lucky enough to have inherited a practice when I can charge top dollar (often to people with more money than brains, sad to say). Thanks again for your kind words. I wish you only the best!

@DR Joey, My deepest regrets on the passing of your mom. I have a feeling she was a great lady and probably you and she were very close. I lost both parents by the time I was 20 my Mom died when I was 17 and my Dad had a heart attack the day after her funeral at 7am and I had to call the ambulance and deal with that .OH boy talk about stages of grief. Well he was hospitalized and they did a heart cath but he never recovered and died right after my birthday Jan 7 Medicine way back then was not what it is now. They both probably would have lived. I too did the same research on Bill and in fact I never fit in with the Natzi's. My family deemed me the black sheep and I was sent from Pa to Florida where I established residency and was able to use my limited trust to go on and graduated from the U of Miami and got introduced to marijuana. My degree was in Business and my thesis was about legalizing marijuana and making it a profit for it. Luckily I had a professor who didn't fall over in shock when I submitted the theme LOL Who would have know I had a vision that is currently happening. But I have always been a bit of a radical and still am. But I don't need to tell you about that controversy. In the last few years over 50% of opiate over doses have been by teenage or early 20's and Caucasian. So yes I agree we have a great deal of public health issues. We need more access to treatment for narcotics and these new so called recreational drugs. If you are an addict get your butt in a good treatment program. Short term detoxes no longer exist, so we have people who spend years on methadone or the wonder drug (sarcastically) Buprenorphine. Buyer beware both drugs are extremely hard to get off of get on and get the hell off. That should be on the label. Another benzo's they are not to be used long term. If you think just because it is prescribed it is ok NOT worse withdrawal than narcotics. So by now you are probably saying how do I know this well folks I have been there done that and regret the day I first stuck a needle in my arm or went to Dr Feelgood and ran out of the pharmacy like I was running from a burning building. Addiction is a disease accept it and treat it as such that deamon chases you all your life and he will chase you forever. Thanks for letting me share a brief part of my story. And remember the suffering addict out there who thinks he or she has control ! May you seek the help you need. peace

@Dr Joey, Wooo Hooo I knew there was a reason I liked you. There should be more Docs like you!!! You overcame your habit and you treat everyone regardless of their sexual preference or addiction. So anyone out there trying to beat the demon listen to Dr Joey the dude knows his s***. Listen to him. Not only has he been there but he could be making major money but volunteers to help addicts. Dr Joey you are the bomb!!!

Yes, these are all public health issues. I"m a gay man and donate a full day to an LGBT Clinic and "terrorize" all available residents who meet the cut (I prefer 3rd and 4th year, not just for experience, but so I don't have to constantly check their scripts) and want a "good later" from me and their own Chiefs to donate as well. Some of my colleagues remind me how much money I could be making having another day of office hours. I'm not here to become the richest man the the cemetery, buddy! As I said, sounds like we'd make quite a team. (And I cut my private fees quite often.)

My friend,

I think I'm bout 10 minutes younger than you are at 60 going on 61. Glad there are no hard feelings and thanks for the kind words. "Thanks" to the AA/NA Cultists, you and I can't call ourselves "addicts" or "recovered addicts" and be taken serious by any professional group. If appropriate, I mention I had a med problem. This incurable Medical Model crap is just that. I've done research that has shocked, appalled and disgusted me, particularly about Bill W. I did a dissertation; if I had time (and I might since I'm retiring) I might shop a dissertation-style book about Bill W., et. al.. The late Lyle Leverich, without any advanced academic background, did the same on Tennessee Williams ("Tom: The Unknown Tennessee Williams") and it was amazingly sourced and accurate. As I said, I wrote a dissertation and know good scholarship. I recognized 20s advertising rhetoric immediately and looked deeper and wider into this cult. I saw what they had planned for me when I returned to college and rejected it completely. I respectfully accept your proposal to agree to disagree about benzodiazepines. Can you recall what it was about? I think writing for Xanax and Methadone. I admit a problem with pregnant women who continue to use and then (a) expect to go back to it; no breast-feeding and/or (b) think they deserve to have their children NOT removed or returned. I've been an Expert Witness God knows how many times and I've never testified on the side of such parents. My personal feelings are that a mother who will knowingly harm a child in utero forfeits her right to the motherhood of that child. Thanks for your kind words. Sounds like we'd make a great team. I'm a great Grant Proposal Writer myself, thanks to my late, beloved, Mom, who passed on May 02, 2015.

He's probably writing for HYDROXYZINE (Vistaril or Atarax are the original brand names). Unless you take at least 100mg (which would dry you out like a SunKist Prune), they're useless. They originally were promoted as tranquilizer; now I THINK hydroxyzine is sometimes used in Labor and that's about it. I've taken it; It's a useless drug. If you have real anxiety, you need real anxiety medication. No one tries to stop shrinks for prescribing anxiolytics (the nose-in-the-air name for anti-anxiety drugs), you might start there. You don't always have to pay a shrink's fees to get ongoing therapy. I have "a bunch" of Master Psychiatric Social Workers (they're changed the initials so often...but I recall MSW - ACSW as the designation in NYS for a Master Social Worker with at least seven years of supervisory experience) as the highest level of psychiatric social worker, absent DSW, of course. They work with me win cases and who do probably 90% of the psychodynamic psychotherapy and I keep an eye on the meds. We work together. I've found a lot of good MSWs hewn it comes to assessment and recommending medications. I may be the MD, but I take my peers' opinions very seriously.

I've suffered from severe anxiety attacks over the past decade, and they are progressively becoming worse. I often feel that I'm having a stroke. Short of breath, locked muscles, and disorientation. But my doctor just prescribes antihistamines....which have zero affect on me.

LOL @Dr Joey, I call them the AA and Na Nazi's. My way or the high way. Don't get me wrong, I know a lot of people who get clean using the 12 steps. I believe that if we use them in our life they can be of great help but after a stint in meetings I discovered it is just a matter of will power. As far as Benzos are concerned I respect your opinion and hope you would afford me the same, But I am 61 and have lived a great productive life. I was the Exec Director of the largest agency in our area. We managed all of the homeless assistance money for our County. I am an excellent grant writer and still hold the record for the largest award to a single agency. I worked on collaboration and Community Building. WE addicts are a talented bunch LOL $435,000 and change from the Pa Commission on Crime & Delinquency But I digress, the money became very political and my contract was not renewed. My successor was a woman who had no clue and the City snagged the money for Police overtime. They did a little programming but not much. We have inner city problems and gang related issues. I run into old clients every day who can not access services.. In my mind all public health issues.. So hopefully this can inspire an addict to use their skills in a positive manner. And we will call our debate on Benzos a stalemate lol peace Doc.

I'm sorry of you interpreted my reply as judgmental. It wasn't. Research has proven that Xanax TENDS to worsen Depressive Illness. That doesn't mean NO NENZOS EVER, like those AA/NA Cultists do. Just suggesting another benzo if possible. I am not judgmental when it comes to addiction. I did my share in college. Two best friends with their Dad's Rx pads...lucky I survived and I might not have if a friend isn't rate me out. I got the help I need (my Dad's a Doc s he said, "NO AA, NO NA. PERIOD. End of conversation." Other groups that did not insist I make a full confession to an unqualified total stranger helped me sit my doing good works at Covenant House when I was a med students. You are being treated HORRIBLY by your so-called Healthcare providers." Any way out f the clutches of the pill pusher. I've hired as residents docs who honestly told me they had drug problem in college or they got busted (state law prohibits licensure if convicted of a Felony) and felt better getting if off their chests right off..."besides, word in school is that you're very understanding". They turned out to be good doctors, deciding to specialize in Substance Abuse, ADDICTION MEDICINE IS NOT A RECOGNIZED MEDICAL SPECIALTY, ASK THE AMERICAN SOCIETY OF MEDICAL SPECIALISTS IF YOU DON'T BELIEVE ME." You deserve better care if ONLY because you''re one of our citizens.

@Dr. Joey, ok so klonopin worked for you and I respect your education, but I have a history of mental illness in my family. I am not going to go into a big sob story. But I am 61 years old and have been to many shrinks rehabs and kicked opiates and methadone. Valium used to be the mothers little helper in the 70's now it is xanax. I was in a head on car accident and prior was diagnosed with degenerative disc disorder. After 20 years of sobriety, I awoke in the hospital on a morphine drip and that awoke the demon lol. I was being treated by a pain management pill pusher and ended up confronting him on my treatment. I accept responsibility for taking the drugs but having pain and PTSD so bad I was reclusive for a year. My husband was seeing him for a work related accident in construction. Well he walked out on me while I was in the hospital. I left AMA stupidly but by then good old pill pusher had me 302 to a facility where I was first put on Bup but that was in 2006 and I live in a area in northeast Pa and the Doctors knew little about bup. So I found a Doctor who was one of the first in my area to prescribe the drug, and tried to transition from the narcotics to Bup, well hello precipitated withdrawal. I was rushed to a hospital in respitory failure. They did nothing. I laid in agony with an oxygen mask praying to die. So the next day he put me on oxys to buy us some time for me to get the pill pushers methadone and other drugs out of my body. It worked until the DEA decided to put him out of business. So I am now on 12 mgs of subutex and xanax. Is it the best protcol in the world, no, but it is mine. So I have only shared a small part of my life. But being judgemental is not a great way to offer support. Walk a mile in my shoes and then comment. I have helped many an addict change their way of life and say Hooray to them. And each day I pray for the suffering addict out there. May they get the help they need. Peace
