Not correct. Buprenorphine was originally created, intended, and fairly frequently used to treat pain. It's most common administration was and is post surgery while the patient is still medically supervised. It was previously administered more liberally in the emergency room setting. For the most part this practice has been replaced by more effective immediate release pure opiate agonist such as Hydromorphone (dilaudid) or fentanyl. This was done mainly because of the antagonist nature of buprenorphine which binds receptors for a much longer duration than the other PO medications I mentioned, making the initial dose of Buprenorphine very effective for treating pain but sequential doses not very useful. Subutex, which does not contain the pure antagonist naloxone/narcan can be a great around the clock pain reliever because of the very long half-life or Buprenorphine. Suboxone however does contain the pure antagonist naloxone, which to a certain degree is counterproductive for treating pain. Suboxone is being misprescribed in another way more so than its prescription for pain. Suboxone was created to be a detox drugs for opiate users with high tolerance and an extremely progressed dependency or addiction. The initial FDA approval was for 5 to 10 day use only, in a medically supervised environment with the first dose beginning 24 to 48 hours after the last pure agonist dose. A gradual decrease of dosage throughout the first 3 to 5 days then a more significant reeducation in dose for days 6 to 10. I am a director and board member for a substance abuse treatment organization in upstate new York, we have several different treatment facilities and at all levels of the continuum of care. This number of clients entering treatment due to Suboxone dependency/addiction has tripled every year for the past four years, so far this year we have more clients with suboxone dependency/addiction than methamphetamine and crack/coke combined. There is no situation when its appropriate for a client to complete a 28 day or more treatment and still be using suboxone as drug replacement therapy. In the field the crackdown on frivolously prescribing suboxone doctors has already begun, this is what happens when doctors prescribe a medication they have little understanding of. Governor Cuomo is pushing to change this practice of suboxone as an opiate dependency drug replacement maintenance therapy back to its original intended use as a detox drug only administered in medically supervised settings.

RPhD I was diagnosed with cancer in April 2015 and I an in excruciating pain that makes me literally sick. Been on suboxone for over 3 yrs. My sub dr won't take me off the suboxone to help me get pain relief. I need a major 10 hr surgery to remove cancer and lymph nodes but the Oncology team is letting my sub dr manage my pain afterwords. He is going to give me 1/2 of an 8mg suboxone for pain every 12 hrs. THEY WONT LISTEN TO ME, 16 MGS A DAY IS NOT HELPING NOW. CAN YOU GIVE ANY ADVICE on how I can get a dr to help me or listen? My oncologist won't even learn about suboxone. I'm scared to death and don't know how much longer I can live in this type of pain. Btw I Live in ky.

I've been on suboxone film 8mg/2mg Sublingual films For a few years now. I am just starting in the past few months to wean down I was on 24 milligrams per day and now I am on 16 milligrams per day. Using this medication for chronic pain has been a wonderous thing for me Although I did not start out taking it for that reason! The amount your body can take Is dependent upon each individual

Your wrong, subutex is not an opiate blocker, because it doesn't contain naloxone like suboxone does. Subutex is a better choice for pain control and it helps people wean off of stronger opiates. Subutex and suboxone are strong opiates. Only suboxone has an opiate blocker. Subutex isn't an opiate blocker. Get you facts straight before giving out such serious advice to people.

Naloxone is in Suboxone, therefore it kills the painkilling properties of the drug! That's the difference between Suboxone and Subutex. So people who say they take it for "chronic pain"! Not true. Just keeps them from " needing" the Oxys, and not get sick. It's used everyday in detox centers for people addicted to opiates.

I was o subutex for over 2 yrs before being put on suboxone and the subutex has a partial blocking effect and binding ability. Every one should do research before assuming they know about it. I went thru a surgery while on subutex and they maxed me out on duladid and morphine ( nurse said she gave me enough to an elephant down) I weigh 100 lbs. If subutex doesnt have a blocker, then all that should have helped me in some way. It did nothing for me! So I did research and talked to some very smart drs. A person has to be taken off Buprenorphine ie.. Subutex/suboxone 2-4 weeks prior to any surgery or any thing where pain meds will be needed.

@fireman625
Suboxone is used for BOTH the treatment of chronic pain AND the treatment of opiate addiction.
Whether Suboxone tastes bad or not is a matter of opinion. I take it and happen to think that it tastes horrible.
How much saliva one produces varies from person to person, and saying that your body produces more saliva from taking the tablets is not an accurate statement. How much saliva your mouth produces is a matter of your body's biology at any point in time. If I just ran a marathon through the desert, and I haven't hydrated adequately, well, then my body might not produce much saliva. On the other hand, if I chug a half gallon of water, well...you get the picture. Sometimes when I take my strips I'll produce a lot of saliva, but other times not.
Not to call you out or anything. Just trying to keep things accurate. :)

You're absolutely right! As a long time detox nurse let them say what they want, bulls*** themselves, there is NO pain killing done with Suboxone, period!!! They just won't admit they are a drug addict trying to prevent withdrawal habits!

The rehab nurse..sooo right. I have to see a new Doctor. .because he writes nothing but..buprenorphine for me and it does not cover my pain..I keep telling him. But he had a patient die in 2013..and now he won't consider writing a real pain rx. Too bad. Buprenorphine has a lot of side effects and I have them. Including the seizures. This is documented!! He threatened me at my last appt. When I brought it up. He said well..I won't write you a script for it or any other opiate .how do you like that??? Considering my diagnosis and the fact that I have been on low dosages of opiates for over fifteen yrs. (Never abused them or lost them or had them stolen) I was stunned. I have 3 appt. With different doctors this month..so I can see a real pain specialist. Who is not known for writing more subutex than any other doctor in the state!! Who also did not receive kickbacks in the amt of 37,000.00 $ for prescribing buprenorphine. Part of the problem?? I asked for my Records. I received incomplete records, that did not include the form I fill out when I see him every month. That form shows my communications with him directly. He listed that I am allergic to medication I am NOT allergic to and some of that medication I have never even taken so I don't know if I am either. He also had me confused with another patient and talked about my knee replacement, which I've never had. He claims he's been counseling me on my weight for 6 month and that I am obese. Which I don't think so because I am 5 foot 7 & I weigh 140 pounds. This guy is such a charlatan. If a doctor is prescribing buprenorphine for pain and you are a chronic pain sufferer who has never had an addiction problem, I don't smoke and I don't drink. Never have and I'm 52 years old. Run and run and run. I let this man blackmail me for so long. Now I'm going to find a real aboveboard doctor and if he has a problem writing a prescription for percocet I'm sorry for him. He will not be my doctor. I do not want to be on a fentanyl patch and I do not want to be on morphine or oxycontin. I just want a low dose to take care of enough of my pain so I can take care of my 94 year old father in law. All my diagnosis is documented and I have been on disability since 2003. This is getting as ridiculous as the oxycontin epidemic was before it. My doctor is not covering his own ass I have read that people are abusing this drug just as much. He most likely may lose another patient. I am glad that he is losing me and not in that way.

32 mg/day is where the ceiling is for increased analgesic effect. After that, it starts to antagonize itself.

I have had rheumatoid arthritis since I was 8 yrs old. Not to mention fibromyalgia, osteoporosis, congestive heart failure and many more secondary diagnosis'. RA is some of the worst pain in the world and my doctor is a renowned specialist in addiction medicine and pain. His name is Dr. Richard Blondell, look him up. Suboxone DOES help with chronic pain and unless you've had severe, chronic pain, how would you know? Do you suffer from pain daily? As a nurse, depending in how long and what you did before, I'm sure you feel the occasional pain or two. If you were ever a CNA, I'm positive you have. How can you just sit there and judge people like that?

I have been on suboxone, subtext, and unsolved... all three in many forms... patches, pills, strips, and troches....for five years... and yes, it does alleviate withdrawal but I does NOT help pain. I have RA and fibromyalgia, ibs, and several injuries from a car accident that caused long-term (permanent) damage. I am miserable, almost every day and in overwhelming pain. But since giving bupe in all its forms the full go I now get no relief from ANYTHING. If there are other treatments available for you, do not go bupe.... You WILL regret it.

Yes but the effects will be minimal for the first few days. Also the risk of overdose is high because subs have a long half life. There is no risk for withdrawal. So whoever said wait 18 hrs is wrong. Now if you took opiates then 2 hours later opiates extreme wd instantly.

You are right to a degree, subutex does not contain the naloxone, however it does still bind to the receptors. I know a lot of people say subutex works for their pain. Well that is fabulous for them. Like I have previously stated, I have been on it for 2 years and every month it's worse. I get more and more of the side effects and less and less of pain relief. I really don't care to wean off opiates personally. I was on them for years and after about 2 years of taking your dose appropriately you no longer feel euphoric effects. I could care less that's not what I wanted them for. I want to be out of enough pain to function in my daily life. I'm still on disability and will never work again. But I would like to have enough pain relief to do my dishes and make dinner just basically sit in a chair and watch TV. I am on my feet all day and night long because it hurts too bad to sit and get up. Is 20 milligrams of subutex a day does not get me through that, then obviously it does not work for me. I don't know what kind of pain other people suffer from. I have fibromyalgia and I have Crohn's disease and I have more than 6 rotated vertebrae I have a disc that are already half gone and I have arthritis to go with it all. Not to mention the pain I have from seven foot surgeries. I have lost 3 inches in height and just 3 years. Now I know there are people out there that have had car accidents where are practically every bone was broken and their body. These people agree with me and say subutex does not work for them also. If I had a history of drug abuse or if I was addicted to drugs I would probably just suck it up and remain on this medication because it would be the safest alternative for me. But I don't have those issues so why should I have to deal with a medication that is a synthetic form of codeine, which by the way I have never taken because it doesn't work. Found that out 40 years ago when I burn my hands and a chemical accident and they gave it to me for pain and it did nothing. I deserve to have a decent quality of life. True I am NOT dying of terminal cancer and I don't need a morphine pump or some ungodly amount of opiates in my system. But I definitely deserve a pain killer that actually works so I'm not experiencing 80% of pain every single day! I want to hear from all the people out there that also do not receive enough pain relief from the subutex. I have heard from people who do and I know that everybody's different and opiates affect everybody in different ways. So I would like to hear from a majority of people that have found the subutex useless as I have. Also I would like to hear about your side effects, some of the side effects with this drug are crazy. The side effects from Buford morphine are hidden from most people because they don't want you to know the truth. Let's start with your ADHD going through the roof if you have it. Then let's move on to the seizures with only this drug have given me. From there we can go on to insomnia where I'm at 38 hours before I can sleep. Oh and my husband's favorite park kiss your libido goodbye cuz that's not coming back. And I lost 100 pounds within nine months of taking it. True I could afford to lose the weight, but I don't like losing weight fast and losing it because a drug wiped out my appetite. There are other little insidious side effects. I used to be a happy cheerful person and now I'm just a little grumpy Gus all the time and I don't like that. It might be due to the increase in pain because its not being treated.. They are telling us get over yourself we're not going to prescribe any opiates. I'm not going to get over myself and I really don't think that for percocets a day at 10 milligrams is over the freaking top for what I have. I was told over 10 years ago that I would be on them the rest of my life. That really sucks but at the same time pain sucks more. Yeah I would love to spend time with my 15 grandchildren but it's kind of hard to visit when you have to sit in a car forever in pain. Or you have to limit your visits because your drugs now have to be filled every month with a written prescription and you cannot have a refill on them. I actually had a written prescription when I visited my grandchildren in Virginia and the doctor accused me of writing it myself. That was funny. I gave him my Dr card and told him to call him and he accused me of making up the card and that one of my friends would answer. That was ridiculous. I said to him call them and talk to him and Latin talk to him as you would talk to another doctor I'm sure you can tell whether you're talking to a doctor or a friend. I would like to thank the DEA for swinging the pendulum so far to the right that nobody can be treated for pain anymore. I do agree that there is a major problem in this country with people becoming addicted to prescription pain pills, but part of me wonders is it really that bad or is that the reason they tell you for all the rehab that have been springing up over the years?? It makes me wonder. Are pharmaceutical companies making so much money on viagra heart medicine blood pressure medicine diabetic medicine and other things that they no longer make anything for pain that really works?? I just don't know anymore. I dunno 4 people in the last 3 months have committed suicide because they could not get their pain under control and their doctors did not listen to them. This breaks my heart and this will never be an option for me. Yet at the same time I find myself laying in bed at night thinking God you know what if I don't wake up tomorrow that might just be okay with me. Well God has other plans cuz I wake up everyday I'm if I never went to bed haha. So I want to hear from people I want to hear honestly from people are you having these nasty little side effects and are you getting any pain relief from subutex?? If you're one of those people that it works for and you don't have side effects like I said I'm extremely happy for you I am but I don't want to hear from you I want to hear from somebody like me. I want to know that I'm not crazy insane. Oh and by the way my husband my family and my friends have said I've lost my damn mind since I started taking this. They have to repeat everything over and over again cuz I forget what they say and I drop stuff all the time. Just saying.
