Yes, it's Teva per Drugs.com. Teva products sold in the US are made in various countries including Israel, India, the Czech Republic, and the US.

Using the link below (which I was directed to from Teva's US site), you'll see near the bottom that Teva metformin is manufactured in India. That seems to be the case for most generic versions of metformin these days.
