New Pain Management Doctor In Bay City, Mi

I'm moving to Vassar, Michigan in a few months and I'm looking for many types of doctors. Bay City, Saginaw, Michigan are some of the bigger city near to Vassar which is a very small city with hardly any doctors. I need a PCP, pain management, ENT, gastroenterologists, neurologist, urologist, nephrologist, hepatologist, cardiologist, and pulmonologist. I'm take 39 medications daily which means I take 87 pills a day plus inhalers, nasal sprays, nebulizer, and inj's. I have been seeing some of these doctors for 20+ years and have great relationships with them. I been seeing the pain management doctor for 7 years. I take Opana ER, oxycodone/apap, Lyrica, and a muscle relaxer. I know not all PM doctors will prescribe the same medications or Narcotic pain medications but I have a clean records with my current doctor. He is a president of this pain management doctor board which means he is legit. He does u/a's at least every other month plus random pill counts sometimes two to three a month or once every other months and never came up short. So it's all in my charts so show I'm not some addict I truly in pain. I get trigger Point twice a month and epidural inj's once a month plus I have spinal cord stimulator implant. So it shows that I'm not just going in for narcotics. My wife is from that part of Michigan and have family up there so my wife grandfather has a lot of health issues so he knows some doctors but if anyone knows any good doctors in those areas please let me know. I'm trying to get enough medications to get me by for the first month until I can get to see these doctors to start receiving these medications from my new doctors. I don't mind leaving my family here in Virginia but starting fresh with all new doctors is kinda of scary. I know they will have my health charts but finding a new doctor and need to gain trust with them and getting a great relationship with them doesn't come easily it take month or years. My current PCP I have seeing her for 17 years and she saw me from a pretty healthy person and my major crash in my health. She doesn't take my insurance anymore so I tried 4 different doctor that does take it and I didn't like none of them so I decide to stick with her and paying a average of $140 a visit verse $15 copay if I found a PCP that takes my insurance. So I'm nervous about moving to a new area and starting over with all new doctors. I'm going 850 miles from where I'm living at now. It's going to be a big change too because I live in a really big city where everything is right down the street to a small town and everything is 20+ miles away other than a dollar general and a gas station and a few other things. The closest Walmart or Target is 30 miles away and there is a Kroger is 15 miles away. So it's going to be a big change. Thanks in advance.
