Both of these pills appear to be exactly the same aside from having different manufacturers.

Both pills contain 10 mg of Methadone hydrochloride. Neither of these are extended release or stronger than the other.

M 57 71 is manufactured by Mallinckrodt Pharmaceuticals.

54 142 is manufactured by Roxane Laboratories, Inc.

I guess everybody is different because the 57 71 lasts longer for me. I found my self taking the 54 142 sooner rather than later compared to the 57 71.

Hi Jeff,

You may also want to take into account your dietary intake before you take these pills. Consuming a meal (prior to taking the medication) that has higher fat percentage, can extended the release of the pill to a small extent. I say this because fat slows down the absorption of nutrients as well as the digestive process that could impact how quickly a pill releases it's contents.

I don't know if you are required to take this medication on an empty stomach or not, but I only presume that your last meal could have an impact, if you haven't digested it yet. Also the time of day that you take the pill would have an impact as well; since there are times when your metabolism may be more revved up than others.

Aside from ruling those situations out, I guess it just comes down to any inactive ingredients in the pill that could differentiate the two brands. I'm not certain what those ingredients may be, if there are any; but a lot of the time there may be inactive ingredients not listed. You'd have to contact the manufacturer's to find out more info on that and whether or not it has an impact on the pills release.

Yeah David's right about the inactive ingredients. Generic drugs, even when they have the same active ingredient, are allowed to vary by a certain percentage from manufacturer to manufacturer ... not to mention inactive ingredient variances. You might try requesting that your prescription be filled from a specific manufacturer, but sometimes the pharmacist can't help it since they only receive what's in the supply chain.

i have been taking methadone for jaw pain for about five years now, from my experience if you switch up every other refill from the round ones to the square ten mg pills it helps make the medicines half life go farther, this is just my personal experience. if you are getting low on pills before your next refill you can take 1 tbsp of baking soda mixed with water about 20-45 min before you take pill and this helps with the half life too. I have been taking an oblong white Methadone 10 mg tablet (54 142) for several years now, it is very bad tasting and it causes problems in my stomach. I was surprised this month when my pharmacy gave me the Roxane Labs 10 mg white round tablet and said they would only have the usual oblong tablet next time so I moved my prescription to a pharm that has it all the time. What makes the (54 142) tab to taste so bad? Is it something to keep the kids away? Or something else? ... P.S. about a year ago I accidentally took all of my tablets at one time and found that I got more mileage from them that way so I made a habit of taking them all at once in the morning ... The Way!

there is a difference between the 2 manufacturers. though i couldnt tell you on a molecular level what th3y are, they effects when switching from one to the other can be felt.. the one you switch FROM usually is perceived to have longer sustaining half life, while the one you switch TO will seem to bring relief quicker and maybe a little stronger pound for pound.. This theory seems to be the case whether switching from round(54's) to rectangular or vise versa. Also whichever methadone (manufacturer) your body recognizes most will seem to work better. if you asked 1000 people which worked better in terms of initial pain management to length of half life, i would expect close to 50-50 would be the case. this would indicate same exact mediction but in reality is more personal preference based on past opioid experience.. also, it seems like a patient coming from oxycodone likes rectangles(57 71) and someone coming from a more sustainable opioid like MS Contin ER seem to favor the round (54's).. this may sound like science or BS, but is only personal experience and opinion.. did i cover it all? any opinions?

I always take all my pills at one time???? I go to pain management doctor and I'm on 100 mlgs and he told me to take them all at once in the morning and I've been doing that for 8 years now I do t care what type they are circle or whatever as long as it's Methadone and my lupus don't kick in.....,

It doesn't come up in a five panel but there is one single test, that a lot of businesses are starting to use and there's a larger panel test I believe it comes up on. My husband and I have both been on Methadone for almost 8 yrs. And at first he didn't have to worry about it but he has since had to tell his company, he almost got fired because of it, and get a letter from the Dr. stating the meds won't affect his work. Which doesn't seem right since it is prescribed. But anyway now a days more companies are testing for Methadone
