Based on the description provided, I found your pill to be Hydromorphone HCL (4 mg).

Hydromorphone is used to relieve moderate to severe pain and severe, painful dry coughing.

My husband had a bad, real bad case of collitis. Some how caused a leak in his colon and almost bled to death. Any way, he was on Morphine and dilaudid. Sent him home with Morphine SUL 15mg Er Tab ENDO taken every 12 hours Started taking 3/23. Also, sent him home with Dilaudid 4mg every 4 hours. Started taking 3/23. Stopped taking both 24 hours ago because blood pressure dropped. Took short time ago and is still low, averaging 80/57. Will it go up, is this the cause? What to do?
